Sports Physicals

                Every athlete trying out, participating or competing must have a physical on file at the school.

Physicals will be given free of charge at BMS on:
April 21, 2009 GIRLS
April 22, 2009 BOYS

1. Any student receiving a physical BEFORE April 20, 2009 will EXPIRE June 1, 2009.
2. Any student receiving a physical after April 20, 2009 will be good through June 1, 2010.
3. All forms should be filled out completely – they are invalid without the proper information.

NOTE:  Physical forms change each year. 
Before scheduling any physical, students must obtain an updated physical form from the school.
